1. Download the Rock Solid Attachments Warranty Claim Form from the link below. The form will open in a new browser window.
   
2. Fill in all fields on the top portion of the form, either digitally or by hand.
3. Send the completed form along with photographs:

• If completed digitally: Attach the form and photos to an email and send to support@rocksolidattachments.com
• If completed by hand: Scan the completed document and email it along with the photos to support@rocksolidattachments.com
